Peer Navigation

Pregnancy - 12 Months Old

Through one-on-one connection, practical guidance, and ongoing check-ins, this service helps families feel less alone and more supported as their needs shift over time. Whether you’re looking for mental health support, community resources, or simply someone to help you think through next steps, Peer Navigation meets you where you are.

  • After enrolling, you’ll start with an intake call with a MomsBloom Navigation Manager. This conversation looks at your current supports, identifies gaps, and explores needs related to both physical and mental health. As part of the intake, two brief, evidence-based screeners are used to better understand how to support you: one focused on perinatal mood and anxiety, and one focused on social and environmental factors that impact wellbeing.

    From there, your Navigator creates a personalized plan that may include referrals to community resources, guidance around postpartum care and mental health, and a check-in schedule that works for you. Check-ins can happen by text, email, or phone.

    Some families choose to complete the intake and receive referrals only. Others continue with ongoing navigation support, with regular check-ins, for up to 12 months postpartum.

    Participants also receive helpful, timely educational emails with information about local resources, urgent maternal warning signs, and opportunities for mental health screening.

  • Peer Navigation is available to parents and caregivers from pregnancy through 12 months postpartum

  • You can enroll at any point during pregnancy or throughout the first year after birth.

  • Peer Navigation focuses on guidance, connection, and resource support without in-home visits. It’s ideal for families who want flexible, ongoing support without scheduled home visits

In-Home Support

Birth - 12 Weeks Old

Our in-home support program pairs families with trained community members and specialists who offer practical help, emotional support, and genuine companionship during the early weeks after a baby arrives. This care is about easing the load and reminding families they don’t have to do this alone.

  • Any family in Kent County with a newborn can receive support—no diagnosis, income requirement, or referral needed. This can be for the first, second, or even seventh child!

  • You can sign up anytime during pregnancy or after your baby is born—the sooner, the better to ensure timely support.

    Requests for Support must be completed before the baby turns 12 weeks old (adjusted age if the baby is premature). We strongly encourage parents to sign up during the second trimester of pregnancy to allow time for our team to ensure we have a trained volunteer in your area by your due date.

  • Yes, we often have a waitlist due to high demand - this support is essential. Matching families with volunteers depends on volunteer availability, family preferences, and prioritizing those with little or no support as well as those at higher risk for postpartum depression and anxiety. We do our best to connect you with support as quickly as possible.

  • Every family is different. In-home support may include emotional check-ins, light household help, caring companionship, or simply having another adult present during the early weeks postpartum. Support is shaped around your needs, your home, and your comfort level.

Make a referral

MomsBloom works alongside community partners to support families during pregnancy and the postpartum season. If you know a parent or caregiver who could benefit from extra support, you’re welcome to make a referral with their consent.

Our services are designed to come alongside families with care, encouragement, and practical help. Referrals allow us to connect families to the kind of support that meets them where they are and complements the care already in their lives.

After a referral is submitted, our team will reach out directly to the family to share next steps and explore which MomsBloom services may be the best fit.
